A New Hope for Fatty Liver Disease: India Approves Wegovy for MASH Treatment

Wegovy, the drug manufactured by Novo Nordisk, has been cleared by India's CDSCO for the use in the treatment of metabolic dysfunction-associated steatohepatitis (MASH) in adults in a historic move for liver disease management. It is an important addition to the indication for the use of Wegovy from chronic weight management and comes as an important milestone towards the solution of one of India's rapidly growing yet neglected health problems.

Metabolic dysfunction-associated steatohepatitis (MASH), previously called Non-alcoholic steatohepatitis (NASH), is the progressed form of Metabolic dysfunction-associated steatotic liver disease (MASLD). It is the condition where excess fats get accumulated in the liver along with inflammation and damage to the liver cells and progresses to fibrosis, liver failure, hepatocellular carcinoma, and cirrhosis when not diagnosed in time. The disease, being silent in nature, goes undetected in patients till its advanced stage.

Metabolic disorders in India are rising due to the rise in cases of obesity, type 2 diabetes, sedentary lifestyles, and poor eating habits among Indians. Such risk factors have led to the rise in prevalence of fatty liver disease, necessitating effective therapeutic interventions to address the situation. Prior to now, treatment was mainly in terms of adopting lifestyle measures such as weight loss, improved diets, and physical exercises, with little pharmacological interventions available for individuals suffering from the condition.

The drug Wegovy is made up of semaglutide, a glucagon-like peptide-1 (GLP-1) receptor agonist whose mechanism of action is similar to the naturally occurring GLP-1 hormone. The drug was initially designed to be used to regulate glucose levels in type 2 diabetes, but it was then approved for use in weight loss. It acts by increasing satiety, delaying gastric emptying, reducing appetite, and resulting in weight loss.

Approval of Wegovy by CDSCO means that it is now officially the first GLP-1 receptor agonist to be approved in India for use by adults with MASH. Research on semaglutide has established its ability to promote better liver histology, reduced liver inflammation, increased chances of MASH resolution besides body weight reduction.

This approval would be highly beneficial to the clinicians as it would provide another tool for helping patients suffering from MASH condition, especially those having obesity and metabolic syndrome. Instead of targeting only the liver disorders, this new drug is designed in such a way that the metabolic dysfunction is addressed which causes the disease to progress. This approach is quite in accordance with the prevailing knowledge that obesity, insulin resistance, and fatty liver disease are closely related to each other and require a combined treatment.

It must be known by the patients that this drug is not the only solution to the problem. The drug is designed in such a way that it could work as a supplement to lifestyle changes. It is also important for the patients to keep in mind that maintaining healthy weight with a balanced diet and physical activities and managing diabetes, hypertension, and dyslipidemia remain an important aspect of MASH treatment.

The approval also holds some significance for the Indian healthcare scenario as increasing awareness about the fatty liver disease along with the improvement in diagnostic techniques implies that there is going to be increased demand for the therapies.

The indication of Novo Nordisk for the drug Wegovy reflects a paradigm shift in modern-day medicine, wherein medicines developed for one illness show promise for a whole host of interrelated metabolic disorders. For those Indians suffering from obesity, diabetes, or fatty liver disease, the approval of Wegovy by the CDSCO provides fresh hope that early treatment will help delay the onset of further complications and lead to improved health outcomes.

In light of the increasing prevalence of metabolic diseases in India, the approval of Wegovy for MASH stands out not just as an important regulatory development, but also as a crucial move towards changing the way fatty liver disease is treated clinically.
